PRIME DRUG PATIENTS: DIRECTIONS FOR NEW PRESCRIPTIONS AND REFILLS

  1. Call another pharmacy in Rhode Island with your prescription information so the pharmacist can call your doctor. You must provide the following:
  • Your name, address, and telephone number
  • Your date of birth
  • Any allergies you have to medications
  • The name of the medicine you are taking and the directions on the bottle label
  • Name and telephone number of the doctor who prescribed it.

OR

  1. Take your prescription bottle to another pharmacy in Rhode Island so the pharmacist can call your doctor;

OR

  1. Call your doctor for a new prescription, which can be called in to a new pharmacy of your choice. You will need the new pharmacy’s name and phone number.

If you have any unused prescription medicines from Prime Drug, crush them and throw them away. Pour liquid medicines into the trash. Do not flush unused medicines. Keep the empty bottle so you will have prescription information that is on the label.
