Promotores de Salud will implement the Club de la Salud program. Promotores will educate the Latino Community on access to healthcare, overweight and obesity and physical activity. The center will provide health education, health referrals and limited health service screenings. Cardiovascular disease is an emphasis of the center. A bilingual nutritionist will provide one-on-one counseling sessions.

Chinese Nurse Association of America (CNAA)
Pawtucket, RI
CNAA will educate the RI Chinese community about physical activities, immunization (including flu shot and pneumococcal vaccination), prenatal care, smoking, sexual health, depression, health care, health insurance, and emergency room use. CNAA will also set-up a free BP clinic in the community.

Center for Hispanic Affairs and Advocacy (CHisPA)
Providence, RI
CHisPA will educate the Latino Community on access to healthcare, overweight and obesity, and physical activity. CHisPA will provide health education, health referrals and health services on cardiovascular disease including high blood pressure, hypertension and stroke, diabetes and cancer.

The Genesis Center
Providence, RI
The "Healthy People at Genesis" initiative will develop and implement programs to improve the health of adult students, their families, and the community surrounding Genesis. The Adult Education classes, Child Care and Early Childhood Education, Intergenerational Literacy Program and Food Service training program will provide instruction on the benefits of physical exercise, overweight and obesity, nutrition, tobacco control, substance abuse prevention, mental health issues, and health care access.

International Institute of Rhode Island (IIRI)
Providence, RI / Statewide
IIRI will provide health education and outreach services to immigrant and refugee families in the area through health fairs, workshops, screenings, and classes focusing on childhood and adult immunizations, nutrition and exercise as related to overweight and obesity, tobacco use, and access to health care. In addition, the project will assist with Rite Care.

JHSH “Minority Family Health Promotion Program” will provide health education, health information and risk reduction activities to children, youth and families served by The JHSH, as well as its staff and volunteers. The program will focus on physical activity, overweight and obesity and mental health.

Central Falls, RI
The Latino Wellness Center goals are to improve health conditions, increase knowledge about access to services, and decrease the number of uninsured Latinos by enrolling families in the Rite care program. Program activities include: community and individual health risk assessments, health education, “Ask the doctor”, screening and referrals, and a community health fair.

PHA’s Health PHASE program will provide health education information and services to adults, including seniors, living in the PHA's public housing developments. Information and services will focus on tobacco use, substance abuse, mental health, immunization and access to health care. Health PHASE will disseminate health education information to adults in the family developments through monthly on-site Health Education Workshops and will conduct periodic health screenings.

Socio-Economic Development Center (SEDC)
Providence & Woonsocket, RI / Statewide
SEDC program will focus on providing health education and disease prevention programs for Rhode Island’s Southeast Asian residents. The goal is for the South East Asian community to live longer, more enjoyable lives by pursuing changes in attitudes, beliefs and behaviors. SEDC focuses on cancer education, cardiovascular disease prevention, diabetes education and disease management. SEDC also encourages the use of primary care and practice of healthy behaviors. In addition, SEDC will conduct cultural competency workshops to further health care provider’s awareness of the Southeast Asian cultures and medical beliefs.

South Providence Neighborhood Ministries (SPNM)
Providence, RI
SPNM will provide a comprehensive health promotion program that will include individual and community health risk assessment, community outreach, health education, consumer empowerment, a health information center, and screening and referral services. The following health topics will be addressed: arthritis, diabetes, cancer, cardiovascular diseases, homicide and intentional injuries, tobacco, mental illness, HIV/AIDS, physical fitness and nutrition.
