In many states, health planning activities can encompass a plan for health status improvement, a plan for health services delivery, or both.

For example, the State of Vermont has both a State Health Plan and a Health Resources Allocation Plan, which are linked:

• Information about Vermont's Health Resources Allocation Plan, which contains a statement of principles, an inventory of existing health care resources, and recommendations for the allocation of resources pursuant to the principles
• Information about Vermont's State Health Plan

The Hawaii State Health Planning & Development Agency has assembled examples of health planning documents from Alaska, Kentucky, Michigan, New Hampshire, and North Carolina. These health planning documents describe the methodology used to determine the need for health care facilities or health care equipment in these states.
