Complete Care Notebook for Raising Children with Special Health Care Needs

The Division of Family Health recognizes the role of families as partners with professionals in the health care of their children. Families are the most important care providers and advocates for their children, and our goal is to assist them in this role. The Division's Office of Families Raising Children with Special Health Care Needs is dedicated to providing families of children with special needs with support, guidance, and encouragement regarding the health care of their child.

This Complete Care Notebook pdf was created in response to requests from families for a portable organizer to record and file their child's important health information, from birth through adulthood. The Complete Care Notebook allows families to easily access information about their child's emergency treatment, health history, care plan, care providers, support resources, and other important information.
