How can families get a home visit?

The Family Outreach Program helps all families get the information and services they need to be as healthy as possible.

• Families may be referred to the program by the hospital at the time of their baby’s birth.Families can be referred by their health care provider or by a community based service provider.
• Families can call the program directly and ask for a home visit.
