Community Partnerships

The Division of Family Health’s Community Partnership Projects develop and support community self-assessments and neighborhood based system improvement plans. They support integrated community investments. Community Partnership Projects have helped core urban communities to assess the status of young children and to implement strategies to ensure that children arrive at school healthy and ready to learn.

  • Child Opportunity Zones (COZs)
    COZs bring schools, families, and communities together to promote success in school for all children. COZs are welcoming places in or near schools where families can receive information, support, services or referrals. They develop state and local partnerships to coordinate resources and services.   • Family Resource Counselors

Family Resource Counselors assist families with enrolling in key programs including RIte Care and WIC.

  • Community Access to Child Health (CATCH)

CATCH projects are funded by the American Academy of Pediatrics to improve medical home access for families and children. Rhode Island CATCH projects have included Washington County, Newport, Providence and Mount Hope.

  • Ready to Learn Providence

Ready to Learn Providence is a broad-based community coalition working to ensure that all Providence children enter school healthy and ready to learn. For more information, see The Providence Plan web site.
