Adolescent Healthcare Transition Program

Office of Special Healthcare Needs Program, Adolescent Healthcare Transition Program assists youth, parents, physicians, schools and other providers in the transition and transfer of youth with special healthcare needs from pediatric to adult healthcare.  Several statewide efforts aim to improve transition outcomes for youth with disabilities and chronic health conditions in RI.  The Department of Health has joined the RI Chapter of the AAP, Rhodes to Independence and Healthy and Ready to Work to implement RI’s three-pronged approach to addressing this issue, including a comprehensive statewide needs assessment, youth and family outreach and education, and healthcare provider training.
