Adolescent Healthcare Transition Program

Youth and Family Resources

Adolescent Transition is a current topic throughout Rhode Island, with each agency and department doing their part to educate and assist youth and their families with the transition into adulthood. Other programs in the state include: RI Department of Education (RIDE), Regional Transition Academies, RI Parent Information Network (RIPIN), RI Department of Human Services (DHS) Office of Rehabilitation Services (ORS), RI Department of Mental Health and Retardation Hospitals (MHRH), and the Sherlock Center for Disabilities. Rhode Island Department of Health Special Healthcare Needs, Adolescent Healthcare Transition Program is bringing the healthcare piece into focus with unique materials designed for youth, their families, educators, and healthcare providers.

  • My Life, My Health Checklists is a three part self-assessment series to be completed by youth and their support system on the stages of healthcare transition including knowledge of condition and medical history, locating and interviewing an adult healthcare provider, etc.

  • Adolescent Healthcare Transition Parent Brochure is a brochure for parents of youth with special healthcare needs introducing Healthcare Transition and describing the steps to increase independence in their youth.
  • Healthcare Transition: Planning for Teens with Special Healthcare Needs is a 4-page booklet for parents, caregivers, or professionals of youth with special healthcare needs describing the importance of healthcare transition and steps to take to be ready.
  • Portable Medical Summary is a summary of important pediatric diagnoses, services, and treatments that would be critical for an adult healthcare provider to know for continuity of care.
  • Benefits at a Glance is a chart outlining assistance programs, provided by the Rhode Island’s Resource Place for Seniors and Adults with Disabilities.
