New Vendor Application Information

Retail grocery stores and pharmacies that would like to become WIC authorized vendors must:

Applicants must meet all eligibility criteria to be considered as an authorized WIC vendor. The number of vendors eligible to participate in the program is limited by WIC guidelines.

Letter of Intent Guidelines

The letter of intent initiates the application process. After the letter of intent is submitted and received by the WIC Vendor Unit, the applicant will be contacted with an initial training date.  Applications will not be accepted until the letter of intent and training are completed.

Reapplication Information

Retail grocery stores and pharmacies that are WIC authorized vendors (that already have an authorized stamp or vendor number) must submit a Reapplication Packet every year. Reapplications will only be offered to compliant vendors in July and must be returned within one month.

Tax Form Guidelines

Tax form T-204 CIG pdf is a state form that all retail vendors submit to the State of RI Division of Taxation each quarter. The WIC Vendor Unit requires that all applicants and reapplying vendors must submit copies of 3 consecutive quarterly T-204 CIG which were previously submitted to the State Division of Taxation as payment. The WIC Vendor Unit requires the copies, front and back, to be an actual copy of the version submitted with the payment.
