Licensure Information and Applications

Full Licensure Requirements

In order to be eligible for a Rhode Island medical license you must meet the following criteria.

If you are a graduate from a medical school in the United States, Canada or Puerto Rico you must have the following:

- MD or DO Degree
- National Exam (eg. NBME, USMLE, LMCC, FLEX)
- Two years of post-graduate training in an accredited program

If you are an International graduate, (a graduate from a medical school outside the United States, Canada or Puerto Rico), you must have the following:

- MD Degree
- ECFMG Certification (Educational Commission for Foreign Medical Graduates)
- National Exam (eg. NBME, USMLE, LMCC, FLEX)
- Three years of post-graduate training in an accredited program IN THE UNITED STATES.

ALL MATERIAL MUST BE COMPLETE AND SUBMITTED TO THIS OFFICE NO LATER THAN 12:00 NOON ON THE DAY BEFORE A SCHEDULED BOARD MEETING IN ORDER TO BE CONSIDERED FOR LICENSURE AT THAT BOARD MEETING.

OF NOTE: A COMPLETED APPLICATION MAY BE DEFERRED BY THE ADMINISTRATIVE STAFF UNTIL THE NEXT SCHEDULED LICENSING COMMITTEE MEETING

Disclaimer 

Please be advised effective October 4, 2007, the application fee for the Rhode Island Uniform Controlled Substance Registration (CSR) has been increased from $100.00 to $140.00.  Also please note that effective November 11, 2007 the initial license application fee will be increased from $437.50 to $570.00.  All applications received after November 11th will be subject to the new fee.
