Hospital Information for the Public
Continuum of Care
The goal of the continuity of care function is to define, shape, and sequence the following processes and activities to maximize coordination of care within this continuum of care.
- Patients have access to the appropriate type of care.

- When the hospital accepts a patient for entry into a particular service or setting, its decision to do so is based on the outcomes of its assessment procedures.
|
- Patients and families receive information about proposed care during the entry process.
|
- The hospital ensures continuity over time among the phases of service to a patient.
|
- The hospital ensures coordination among the health professionals and services or settings involved in a patients care.
|
- The hospital provides for referral, transfer, or discharge of the patient to another level of care, health professional, or setting, based on the patients assessed needs and the hospitals capacity to provide the care.

- The hospital ensures that appropriate patient care and clinical information is exchanged when patients are admitted, referred, transferred, or discharged.
|
- An established procedure(s) is used to resolve denial-of-care conflicts over care, services, or payment.

Continuum of Care
This area reflects an evaluation of how the hospital facilitates the patient's access to the appropriate level of care; the right health care providers; and the correct setting and services to meet the patient's needs. Surveyors evaluate how well the hospital provides access to care; how well services are coordinated to ensure that the patient receives appropriate care from entry through discharge; whether the hospital helps the patient plan for care after discharge; and
- whether the hospital's decisions about continuing care, referral, transfer, and
