Individualized Care Pilot for Nursing Homes

Background:  The Rhode Island Department of Health Office of Facilities Regulation conducted an Individualized Care Pilot (ICP) as part of the standard federal and state survey at nursing homes between November 1, 2007 and April 30, 2008.

The Individualized Care Pilot was developed in collaboration with the Centers for Medicare and Medicaid Services (CMS), the Commonwealth Fund, and with HEALTH’s educational partner for the project, Quality Partners of Rhode Island.  It promoted individualized, resident-centered quality of care and quality of life for nursing home residents by:

  • The Individualized Care Pilot was developed in collaboration with the Centers for Medicare and Medicaid Services (CMS) and the Commonwealth Fund to promote individualized, resident-centered quality of care and quality of life for nursing home residents by:
  • Disseminating understanding and awareness among nursing home providers, survey agency staff, nursing home residents, and other stakeholders of how federal and state regulations support and mandate implementation of individualized, resident-centered care in order to achieve the highest practicable physical, mental, and psycho-social well-being for each nursing home resident;
  • Supporting implementation of practices that meet regulatory requirements and accomplish individualized resident-centered care in three key areas:
Resident-Directed Choice;
Staff-Resident Relationships; and
Personalized Environment.
  • Survey Forms:  Effective May 1, 2008, the Office of Facilities Regulation incorporated many of the interview questions and protocols developed during the ICP into the nursing home survey.  All of the questions/protocols directly relate to existing regulatory expectations that support resident-directed choice and a homelike environment.  Forms that the surveyors will be using can be viewed here.
  • The survey forms that were used as part of the pilot are available for viewing as well.
  • Educational Materials: The educational materials that OFR provided to homes during the survey phase were developed in collaboration with the project’s educational partner, Quality Partners of Rhode Island.  They are available on this website for free download. In addition you are invited to submit and view ideas submitted by facilities to promote individualized, resident-centered care.

Please periodically refer to this website for additional materials as the final products and findings of the ICP are publicly reported, as well as for resources to support your facility in implementing individualized care practices.
