Radiological Health Program

Radioactive Materials (RAM)

General Licensing

Submitted GEN-1 thru GEN-4 application forms are assigned a GEN-number.  The returned application, which identifies the GEN-Number, is the Registration Certificate:

  • Agency Form GEN-1 ”Registration Certificate-Use of Depleted Uranium Under General License.” Initial filing with re-filing when information has changed _no fee required.
  • Agency Form GEN-3 ”Registration Certificate-In-Vitro Testing with Radioactive Material Under General License.” Initial filing with re-filing, when information has changed _no fee required.

Agency Form GEN-4-Series Registration Certificate-Annual Registration and update is required to account for certain Isotopes in “Quantities of Concern that are Generally Licensed.” (An initial registration fee is required per facility location)

Specific and Reciprocity Licenses and Disposal of RAM Form

Completed licensing forms, supplemental documents and licensing fees must be submitted to initiate licensing process. A disposition of material and supplemental documents must be submitted to initiate termination of a license.

Licensing fees are identified in PART I of the regulations.  Amendment fees are assessed based on the type of amendment action requested. The licensee will be invoiced for the applicable fee if required.

Radioactive Materials Licensing Regulatory Guides

Regulatory guides are provided to assist the applicant in completion of applicable licensing documents.  Licensees are directed to RI 1556-Medical for completion of MAT-2 applications.

Orphan Source And DOT Exemption

Waste transfer and scrap metal facilities often have “Portal Monitoring Devices” to detect the presence of radioactive material in shipments delivered to their facility.  There are specific procedures to handle responses to radiation alarms and documentation that is required to return the suspected shipment to its place of origin.  The Radiological Health Program must be contacted to issue a “DOT Exemption” before the shipment can be returned. 

 

 

 

Our Mission

The primary mission of the Program is to assure the safe use of radioactive materials, x-ray equipment, and tanning equipment in order to protect the Rhode Island public and workers from excessive exposure to ionizing and non-ionizing radiation through a licensing and targeted enforcement program.
