X-Ray Registration

The applicant should select the appropriate registration form based on the “type of use” of the x-ray equipment or services provided.  The applicant will be invoiced for the appropriate registration fee based on the application and type of information submitted.  For those registration categories other than Diagnostic X-Ray the applicant can submit the registration fee in conjunction with the application.  Registration fees are identified in PART I of the regulations.

The application form can also be used to address amendments to registrations that currently exist, e.g.: to change information currently identified on the Certificate of Registration.
