Office of Health Professionals RegulationInstructions for Completing "Online" HEALTH License Applications:Our Adobe Acrobat Fill-in pdf forms DO NOT allow you to "apply online" for a Rhode Island Department of Health license/certification. They DO, however, allow you to enter the information into the application form while it is displayed on your computer screen and to print out a completed form for mailing to the appropriate Board/Office. The forms provide a high quality printout for your records and are easier for the Department of Health to process. PLEASE NOTE: Although MOST of the form can be completed in this manner, certain pages MUST be completed by hand (e.g. Interstate Verification Forms (if applicable), any pages requiring signatures, etc.). In addition to these instructions, you MUST read and understand all instructions that are contained within the application itself. You are responsible for the completion of the application. All sections/questions must be answered completely (or marked with "NA" if not applicable). IF THERE IS A QUESTION WITH A YES OR NO ANSWER, YOU MUST CHECK EITHER YES OR NO - WE ARE UNABLE TO ACCEPT ANY YES OR NO QUESTIONS MARKED AS "NA". The application must be completed, and appropriate fees provided, for your application to be processed. Incomplete applications will be returned to you. TIP: Print a blank copy of the license application for your review and familiarization with questions and layout prior to online completion. SPECIAL NOTE REGARDING HYPERLINKS: WebLinks are contained in HEALTH Applications that will direct you to various web pages pertaining to your profession (Rules & Regulations, Statutes, Professional Organizations, etc.). If you open the application within your web browser (rather than saving the file to your computer), please visit theses links PRIOR to entering any information into your application, otherwise the information that you have entered may be erased upon returning back to the Application. Form Instructions: 1 Select the “Hand Tool". 2 Position the hand tool pointer inside a form field, and click. The hand tool pointer will turn into an "I-beam". The I-beam pointer allows you to type text (The hand tool also allows you to select a button, a check box, a radio button, or an item from a list). 3 After entering text or selecting an item, check box, or radio button, do one of the following:
- In a multiline text form field, Enter or Return goes to the next line in the same form field. - You can use your mouse pointer to scroll through pages and fields as necessary. 4 Once you have filled in the appropriate form fields, use your mouse to select an area of the form outside of a form field before attempting to print the form. This is important because if a form field is active (has a blinking bar in it), the contents of that field will not print. If the fill-in form is displayed within your web browser’s window, it is recommended you use the printer button on the Acrobat toolbar menu to print the form instead of your web browser’s print function. Print the form and manually complete the remaining sections for submission to the appropriate Board/Office (Address listed in Application Instructions). IMPORTANT: You will NOT be able to save a completed fill-in form with Reader software. You can print the form and look it over to verify your entries prior to closing Acrobat Reader. TOOLBAR TIPS: You can use the "Zoom In" Tool (Magnifying Glass + & -) to enlarge sections of the form for easier viewing (Return to normal size by right-clicking your mouse and selecting a new view). The "Find" Tool (Binoculars) can be used to search any text in the form.
