Document Request

The Office of State Medical Examiners is committed to providing families with accurate documentation. Autopsy reports are available upon request to the next of kin. A copy of a death certificate is available through your funeral home or through HEALTH's Office of Vital Records (222-2812).

To obtain a copy of an autopsy report, the next of kin must submit a letter in writting indicating the decendent's name, date of death, and a check for $40.00 made payable to the General Treasurer of Rhode Island to the follow address.
