The Governor’s Awards for Wellness Innovation

 

Welcome to the 2008 Governor’s Awards for Wellness Innovation Application System

 

Please note: This site has intermittently been experiencing technical difficulties. If you have difficulty submitting your application online please complete the Application Worksheet (available in Word file below) and email it to Erin.Brown@health.ri.gov

 

Step 1:  Determine who should fill out the application.
The person filling out the form must be affiliated with the organization that is applying for an award.  If you believe that an organization is deserving of being recognized, but are not affiliated with that organization, please recommend to them that they apply.

Although anyone affiliated with an organization can fill out an application for that organization, it is recommended that the person closest to the program or policy (the one who developed it or ran it) fill out the application to ensure the most complete and accurate responses. 

 

Step 2:  Obtain approval from the head of the organization.
The head of the organization (CEO, principal, executive director, etc.) must approve the applications.  This ensures that each organization applies only once and puts forward their best program or policy for consideration.

 

Step 3:  Download the application questions.
Applications must be filled out all at once online, so please download the application worksheet.   This worksheet lists all the application questions and will allow you to prepare your responses in advance.  Once you start the online application process, you will not be able to go back in and edit or finish your application.

 

Step 4:  Fill out the online application.

Use your worksheet to fill out the online application. ALL applications must be received by midnight (11:59pm EST) Sunday, August 31, 2008.
