2009 Governor’s Awards for Wellness Innovation Application

 

Please click here for application, download, complete and mail to Cheryl Martone at Office of the Governor, Room 115 State House, Providence, RI  02903. You may also e-mail your application to Cheryl at cmartone@gov.state.ri.us in Word 97-2003 format.   Before you start your application:

Step 1:  Determine who should fill out the application.
The person filling out the form must be affiliated with the organization that is applying for an award.  If you believe that an organization is deserving of being recognized, but are not affiliated with that organization, please recommend to them that they apply.

Although anyone affiliated with an organization can fill out an application for that organization, it is recommended that the person closest to the program or policy (the one who developed it or ran it) fill out the application to ensure the most complete and accurate responses. 

Step 2:  Obtain approval from the head of the organization.
The head of the organization (CEO, principal, executive director, etc.) must approve the applications.  This ensures that each organization applies only once and puts forward their best program or policy for consideration.

Step 3:  Download and Complete the application.
If you email your application, please complete it in Microsoft Word 2003 or convert it to Microsoft Word 2003 or a .pdf file before sending. If that is not possible please send the printed application by mail. Remember - 50% of your score will be based on innovation and creativity. Click here for scoring guidelines. ALL applications must be received by midnight (11:59pm EST) Sunday, August 31, 2009.
